ELITE ZEBRAS
TEAM WAIVER & RELEASE OF LIABILITY
In consideration of being permitted to participate in any Elite Zebras event, tournament, clinic, training session, or related activity (the “Event”), the undersigned Team Representative, on behalf of the entire team (including all players, parents/guardians, coaches, managers, trainers, and team personnel), acknowledges and agrees to the following:
1. Assumption of Risk & Release
The Team Representative understands and acknowledges that participation in hockey and related activities involves inherent risks, including but not limited to injury, illness, disability, property damage, or loss.
On behalf of the entire team, the undersigned fully releases, waives, and holds harmless:
Elite Zebras Referee Association
Elite Zebras Training Academy
All Elite Zebras events, tournaments, and programs
All affiliated organizers, officers, directors, and representatives
Officials, referees, instructors, evaluators, and coaches
Agents, employees, and volunteers
Arena owners, operators, and staff
Sponsors, partners, hotels, and advertisers
Other participants and teams
from any and all claims, demands, actions, or liability for injury, disability, death, loss, or damage to person or property, whether caused by negligence or otherwise, arising from or related to participation in the Event.
2. Insurance Requirements
The Team Representative confirms that:
The team carries valid and active liability insurance with a minimum coverage of $2,000,000
All participants (players, parents/guardians, coaches, managers, trainers, etc.) carry valid medical and accident insurance
All insurance policies are fully paid, current, and in good standing
3. Acknowledgement of Risks Including Health & External Factors
The Team Representative acknowledges that participation may involve risks beyond the control of the organizers, including but not limited to:
Communicable illnesses (including COVID-19 or similar concerns)
Facility-related risks
Travel or accommodation-related issues
Weather conditions or environmental factors
Other unforeseen circumstances
The undersigned agrees to assume all such risks on behalf of the team.
4. Event Changes, Cancellation & Refund Policy
The Team Representative understands and agrees that:
Events may be postponed, rescheduled, or modified due to unforeseen circumstances
Entry fees are non-refundable unless otherwise stated in writing
Where applicable, a credit toward a future Elite Zebras event may be issued
5. Media Release & Consent
The Team Representative, on behalf of all team participants (including players, parents/guardians, coaches, and staff), acknowledges and agrees that:
Photography, video recording, and other media may be captured during any Event
Elite Zebras Referee Association and Elite Zebras Training Academy retain full rights to use this media
This includes use for:
Marketing and promotional materials
Social media platforms
Website and digital content
Print materials and sponsorship promotions
The undersigned grants full, unrestricted, royalty-free permission for the use, reproduction, and distribution of such media without compensation or further notice.
The Team Representative confirms that all participants have been informed of and consent to this media release.
6. Authority & Agreement
By signing below, the Team Representative confirms that:
They have full authority to act on behalf of the team and all participants
All participants have been informed of and agree to these terms
This agreement is binding on all members of the team
